The Community Profile is a compilation of the health and wellness needs from the perspective of Grant County residents. Over 5,000 adult residents participated in the Community Assessment Survey in 2012, breaking New Mexico's record for participation previously held by Bernalillo County.

The residents' resulting issues are priorirized by the Health Council, which then gathers community leaders to address and create a Community Health Improvement Plan for our County's unique needs. 

By demonstration the residents' needs and planning for change, the assessment and resulting documents help to leverage tens of millions of dollars for our community in programs and services requested by our residents. The 2008 Community Assessment leveraged $15 million for services and programs. Corre Cominos Transit System, Gila Regional's First Born Program, the Volunteer Center of Grant County and many other services are the direct result of the Health Council's Community Assessment and Community Health Improvement Plan. 

The 2012 Community Survey, and its results, are available for download in the Community Profile 2013. The Community Health Improvement Plan is currently being drafted, and will be available soon. Look below for some of the highlights of the Community Assessment Survey 2012, Your Home, Your Voice!
